ICD Need to know how to code Malignant ascites primary site unknown

I having a disagreement with my fellow coder. It states under 789.59 that you are to code the primary and/or secondary malignancy first and it states 197.6 for the secondary malignant neoplasm of retro-peritoneum and peritoneum. So I think its 197.6 and 199.1? please help?

I might be a little confused. 789.59 is ascites which is not Ca.
789.59 - Other ascites

So, the first thing I would ask myself is what is the Pt. being seen for? Is it 789.59 or the Ca?

789.59, 197.6, 199.1 tells me......
The patient is being treated for ascites and happens to have a secondary Ca of the peritoneum but the primary Ca is unknown at this time.

197.6, 199.1, 789.59 tells me......
The patient is being treated for secondary Ca of the peritoneum with the primary Ca unknown and the pt. also has ascites.
